Home health software where the visit, the EVV record and the claim are one record
AgenciesForge runs a home health agency's visits, EVV, OASIS-E2 assessments, plans of care and billing on one record. Nurses complete OASIS by conversation with an AI assistant that pre-fills from the chart, and every item is confirmed by the clinician before it is saved. Visits go to your state's EVV aggregator, and Medicare billing (Notice of Admission, PDGM, 837I final claims) and Medicaid claims go out through the included clearinghouse. Unlimited clinicians on every plan; certified agencies start at $199 a month.
Setting up is free. You pay when you go live. Unlimited clinicians on every agency plan.
See what iQIES would reject, before you sign
The OASIS review surface: what the visit captured, what is still open, and the exact items that would come back as errors — checked against the CMS spec locally, while you can still fix them.
Recorded in the product. Demo agency, fictional patient.
What a home health agency gets
OASIS-E2 by conversation
The assistant asks each item in plain language, pre-loaded with the patient's diagnoses, medications and prior scores. The clinician confirms every answer. Review and Sign shows what iQIES would reject before signing, then exports the CMS-format file.
Plan of care, orders and face-to-face
The 485 plan of care, face-to-face tracking, physician orders with an aging list of orders not yet returned, and recertification timing.
EVV that matches the claim
Check-in and check-out on the visit, sent to Sandata, HHAeXchange or your state's system. A visit that fails EVV is caught before it becomes a denied claim.
Scheduling by fit
Suggests the clinician for each visit by credentials, skills, language, distance and overtime, and keeps continuity with the patient's usual staff.
Medicare and Medicaid billing
Notice of Admission, PDGM grouping, a final-claim readiness check, 837I submission, eligibility checks and ERA posting. The clearinghouse is included.
Therapy visits
Therapy documentation with the reassessment clock tracked, so a missed reassessment shows up before the visits after it become unbillable.
What to check in any home health software
Whichever product you choose, these decide how much billing you lose:
- Does it connect to the EVV aggregator your state and payers use, or do visits get re-entered in a second portal?
- Is it on the current OASIS version, and does it validate the assessment before you sign?
- Will it hold a final claim until the plan of care is signed, the face-to-face is on file and orders are back?
- Is the clearinghouse included, or a separate contract?
- Is it priced per clinician? Home health staffing moves with census; a per-seat bill moves with it.
Pricing
| Plan | Per month | Active patients | Clinicians |
|---|---|---|---|
| EHR Starter | $199 | Up to 50 | Unlimited |
| EHR Professional | $499 | Up to 200 | Unlimited |
| EHR Enterprise | $1,099 | Up to 500 | Unlimited |
No setup fee on these plans, and the clearinghouse is included. A new agency still working toward Medicare certification can start on a certification plan, which walks the state license, CMS-855A, accreditation and initial survey steps.
Is it the right fit?
A good fit
- Medicare-certified home health agencies billing Medicare and Medicaid
- Agencies in states with an EVV mandate for home health visits
- Agencies that also run personal care or waiver services on the same staff
- New agencies working toward Medicare certification
Probably not a fit
- Hospice-only providers
- Agencies serving more than 500 active patients (talk to us about a custom plan)
Questions owners ask
What is the best EHR for a home health agency that needs EVV and Medicaid billing?
Pick one where the visit, the EVV record and the claim are the same record, so a visit that fails EVV can't slip into a claim. AgenciesForge works that way: visits are scheduled and verified in the platform, sent to your state's EVV aggregator, and billed through the included clearinghouse. Established alternatives include HHAeXchange, Axxess, WellSky, Alora, AlayaCare and AxisCare.
Does it support OASIS-E2?
Yes. The nurse completes the assessment by conversation with the AI assistant, which pre-loads diagnoses, medications and prior scores from the chart. Every item is confirmed by the clinician before it is recorded, and a review screen shows anything iQIES would reject before you sign.
Can it submit OASIS to iQIES?
It produces the CMS-format OASIS file, validated before signing. CMS offers no submission API for iQIES, so the file is uploaded in the iQIES website, as it is for every vendor.
Which EVV systems does it work with?
It has connections for Sandata, HHAeXchange and state-run EVV systems, plus its own EVV where a state allows an alternate system. Which one applies depends on your state and payer; we confirm the connection with you during setup.
Does it bill Medicare home health?
Yes. It files the Notice of Admission, groups the period under PDGM, checks the final claim for readiness (signed plan of care, face-to-face, returned orders), and submits it as an 837I through the included clearinghouse.
Does it charge per clinician?
No. Every agency plan includes unlimited nurses, therapists and aides. The price is set by active patients: $199 a month for up to 50, $499 for up to 200, $1,099 for up to 500.
Can we try it before paying?
Yes. Setting up is free: add staff, enter patients, connect billing. You pay when you go live, meaning when you start documenting visits, running EVV or submitting claims.
